Experiencing overwhelm or cognitive decline due to aging, injury, or illness can feel frightening—especially if one doesn’t have family or trusted people available to help. At Patient Advocates, we provide compassionate support to help Canadians manage healthcare, daily life, and living arrangements with confidence and dignity.

Our services are designed to ease stress, simplify decision-making, and restore a sense of control and joy in day-to-day life. Below, you’ll find an overview of our services, so you can choose the level of support that best fits your needs.


  • This confidential consultation includes intake, and the provision of professional recommendations and resources for clients to pursue themselves, if capable.

    Services include:

    - Comprehensive intake assessment to gather all essential health information, and to determine needs and goals of the patient and care partners

    - Research performed to address and provide solutions for client concerns and challenges

    - Followup to provide recommendations + resources to support the client going forward

    3-5 hours | $95/hr

  • Working together to implement recommendations and attain goals discussed during the initial consultation by providing expert follow-up support and, if needed, ongoing patient care management.

    Services may include:

    • Assisting patients in understanding their diagnosis and treatment options, as well as navigating complex medical systems

    • Resolving issues, addressing concerns, and advocating for the patient's rights and preferences, ensuring they receive appropriate care and treatment

    • Acting as a liaison between the patient, family, and all members of the healthcare team, ensuring clear and effective communication, understanding, and collaboration

    • Procurement of resources, services, and tools that will enable patients to understand their health conditions, treatment options, and self-care strategies

    • Engaging in open and practical financial discussions to help determine the affordability of outside services, offering perspective, examining budgets, and suggesting ways to navigate available options to access what’s needed for the best possible quality of life

    • Facilitate and access home care services to allow clients to age in place, and stay at home. Communicate with AHS to access quality home care and explain funding model options that could work for best for the client.

    • Assisting patients and families in decision-making and providing the appropriate official documentation with respect to end-of-life care, advance care plans, and/or MAiD requests, ensuring their directives are carried out when the time comes

    • Offering emotional support, empathy, and encouragement


    $95/hr | Retainer required

  • Two powerful tools that help ensure a patient’s medical and end-of-life care preferences are known, and respected, are a Personal Directive - a legal document containing health directions for when one can no longer speak for oneself - and a Letter of Wishes for medical care. Having these in place will greatly reduce uncertainty, and provide peace of mind for both you and your loved ones.


    We provide one-on-one assistance and document preparation for:

    • Personal Directives with comprehensive, legally binding health directives

    • Letter of Wishes for medical care

    Our sessions provide personalized, comprehensive support for making clear, informed decisions about your wishes such as preferences for resuscitation and treatment, who will speak for you in a crisis, organ/body donation, etc. - and the corresponding legal documents necessary for ensuring those wishes will be honoured.

    We also offer assistance in understanding and establishing end-of-life preferences like Expected Death in the Home (EDitH) & Medical Assistance in Dying (MAiD), and a referral to a trusted professional to complete, if desired.


    3-4 hours | $95/hr.

  • Applying for disability benefits or financial support can feel overwhelming—especially while also managing health concerns. Our caring team is here to help make the process easier, guiding you with compassion and clarity every step of the way.

    Our services include:

    • Determining eligibility for a wide range of provincial, federal, and private programs, including AISH, CPP Disability (CPP‑D), the Disability Tax Credit, the Canadian Dental Care Plan, Workers’ Compensation (WCB), employer disability plans, and private insurance.

    • Application assistance—helping you complete and submit all forms accurately and on time, and ensuring that all required documents are collected and communicated effectively.

    • Medical coordination—working closely with your doctors and healthcare team so your medical information clearly reflects how your condition affects your daily life and ability to work.

    • Appeal and denial support—if your claim has been denied, we explain the reasons clearly, outline your next steps, and help you prepare a strong appeal package. When needed, we can also connect you with trusted legal partners for additional support.

    • Registered Disability Savings Plan (RDSP)—offering information and assistance to set up an RDSP so you can access available government grants and start building long‑term financial security (no personal contribution required).

    Our goal is to ease the burden of the process—so you can focus on your health while we help you secure the financial support you are entitled to.


    $95/hr | Retainer required

  • Periods of transition can be stressful for anyone, especially for those managing chronic or complex health issues. A professional organizer can help with the overwhelming process of downsizing your home, making life more manageable. We do not tell our clients what to let go of or what to keep; we guide and advise on the best ways to minimize possessions, then do the heavy lifting!

    Services may include:

    • Assistance with the decision-making process of what to keep and what to let go of, as well as with selling, donating, and disposal of what is no longer needed

    • Packing and coordinating the move

    • Furniture placement and organization in the new residence

    • Organization of digital and paper systems, to ensure needed documents are readily available for appointments and any medical emergency

    • General life organization, decreasing overwhelm and facilitating aging in place safely


    Min. 3hrs | $70/hr | Retainer required
